#MERS #Coronavirus Saudi Arabia - 2nd Death in Medina - 49 Total Deaths

A 55-year-old Jeddah man suffering from renal failure died of complications from Middle East Respiratory Syndrome (MERS) at a Madinah hospital. His death brings the number of patients worldwide killed by the virus to 49.
Forty-three people have died of MERS in Saudi Arabia. The total number of cases in the Kingdom is 84.
Earlier this week, the Health Ministry reported that a 38-year-old Saudi, who was suffering from severe pneumonia and respiratory failure had died.
The victim in Madinah was a Saudi originally from Jeddah, making it the second reported MERS case from the city. A 2-year-old child died early in July.
The man was also suffering from chronic renal failure and was undergoing treatment at the ICU at King Fahd Hospital.
Meanwhile, Dr. Halden S. Shane, president and CEO of TOMI Environmental Solutions, Inc., of Beverly Hills, California, told an environmental and medical group at an infectious diseases conference in Riyadh that binary ionization technology could be effective in combating bacteria and viruses.
Binary ionization technology is a disinfection system that uses cold plasma to a hydrogen peroxide-based aerosol. The application could be effective against MERS, Shane told the group in Riyadh.
According to the WHO, 94 laboratory-confirmed and 16 probable cases of the MERS have been reported to the organization since April 2012. Other affected countries in the Middle East include Jordan, UAE and Qatar.

Ziad A. Memish, MD, Saudi Arabia's deputy minister for public health, discussed MERS-CoV during a webinar presented today by the Emerging Pathogens Institute at the University of Florida. He covered much of the same ground as in an Aug 21 webinar from Washington, DC, but offered a few new details.
Memish is part of a team of scientists who have hunted for the source of MERS-CoV by testing bats and other animals in Saudi Arabia. Last week the group reported that a small fragment of DNA from a bat near the home of the country's first MERS-CoV victim was a perfect match for the human virus (see today's related story).
He said the team, which includes scientists from Columbia University and the Ecohealth Alliance, also looked for the virus in camels.
"We did not find the virus in any of the camels we tested," Memish said, but he did not disclose how many camels were tested or what tests were used. He indicated that the findings would be published soon.

WHO: Middle East respiratory syndrome coronavirus (MERS-CoV) - update 8/30/13

 WHO has been informed of an additional four laboratory-confirmed cases of Middle East respiratory syndrome coronavirus (MERS-CoV) infection in Saudi Arabia.
The first patient is a 55-year-old man with an underlying medical condition from Medina who became ill on 17 August 2013 and is currently hospitalised.
The second patient is a 38-year-old man with an underlying medical condition from Hafar al-Batin who became ill on 8 August 2013 and died on 17 August 2013.
The third and the fourth cases are family contacts of the second patient. Both the cases, a 16-year-old boy and a seven year-old girl, tested positive for the MERS-CoV virus. They are both healthy and do not have any symptoms of illness.
Globally, from September 2012 to date, WHO has been informed of a total of 108 laboratory-confirmed cases of infection with MERS-CoV, including 50 deaths.

Neuraminidase inhibitors are the only licensed antiviral medications available to treat avian influenza A(H7N9) virus infections in humans. According to a neuraminidase inhibition assay, an R292K substitution reduced antiviral efficacy of inhibitors, especially oseltamivir, and decreased viral fitness in cell culture. Monitoring emergence of R292K-carrying viruses using a pH-modified neuraminidase inhibition assay should be considered.

CDC Emerging Infectious Diseases: Antigenic and Molecular Characterization of Avian Influenza A(H9N2) Viruses, Bangladesh

Abstract

Human infection with avian influenza A(H9N2) virus was identified in Bangladesh in 2011. Surveillance for influenza viruses in apparently healthy poultry in live-bird markets in Bangladesh during 2008–2011 showed that subtype H9N2 viruses are isolated year-round, whereas highly pathogenic subtype H5N1 viruses are co-isolated with subtype H9N2 primarily during the winter months. Phylogenetic analysis of the subtype H9N2 viruses showed that they are reassortants possessing 3 gene segments related to subtype H7N3; the remaining gene segments were from the subtype H9N2 G1 clade. We detected no reassortment with subtype H5N1 viruses. Serologic analyses of subtype H9N2 viruses from chickens revealed antigenic conservation, whereas analyses of viruses from quail showed antigenic drift. Molecular analysis showed that multiple mammalian-specific mutations have become fixed in the subtype H9N2 viruses, including changes in the hemagglutinin, matrix, and polymerase proteins. Our results indicate that these viruses could mutate to be transmissible from birds to mammals, including humans.

Geneva/Abu Dhabi: An Emirati with a previously confirmed case of Middle East Respiratory Syndrome Coronavirus (Mers-CoV) has died, the World Health Organisation (WHO) said. The 82-year-old Emirati, who had been undergoing chemotherapy for the last two years, was the first death from Mers coronavirus confirmed in the UAE. The world health body’s report on Wednesday said the Emirati man died earlier this August and was not known to have travelled abroad. Susceptibility to the deadly virus increases when a patient has pre-existing conditions.

I have cleaned up my list, thanks to the information from the World Health Organization and Saudi Arabia's disclosure.  Here is a listing of some of the latest cases, from August 21st to present.  The case in Qatar, 59(M) has traveled to Medina Saudi Arabia before getting sick.  We had a case reported Wednesday (8/28) from Medina. The case we reported, is stated as contracting the virus from the "Eastern Province".  So if the Qatar case were to contract it from the case reported in Medina, the Medina case is reported to have caught it from the Eastern Provnce.    I do not usually put up "suspected" cases.  There is one on the list below.  The family of confirmed cases are always checked as part of the protocol, and KSA has been doing an excellent job.

WHO Middle East respiratory syndrome coronavirus (MERS-CoV) - update August 29, 2013

 WHO has been informed of an additional two laboratory-confirmed cases of Middle East respiratory syndrome coronavirus (MERS-CoV) infection in Qatar.
The patients include a 59-year old man with an underlying medical condition who became ill on 15 August 2013. He is currently hospitalised and is in stable condition.
Preliminary epidemiological investigations reveal that the patient travelled to Medina, Saudi Arabia for 6 days and returned to Qatar on 15 August 2013. He did not take part in Umrah and did not visit to Al-Masjid an-Nabawi in Medina. Further investigation is on-going.
The second patient is a 29-year-old man with an underlying medical condition who had no history of recent travel outside the country.
The results of both the cases were confirmed by an international reference laboratory. A total of 138 healthcare workers, family and community contacts have been screened in the country and so far all tested negative for MERS-CoV infection.
Globally, from September 2012 to date, WHO has been informed of a total of 104 laboratory-confirmed cases of infection with MERS-CoV, including 49 deaths.
Based on the current situation and available information, WHO encourages all Member States to continue their surveillance for severe acute respiratory infections (SARI) and to carefully review any unusual patterns.
Health care providers are advised to maintain vigilance. Recent travellers returning from the Middle East who develop SARI should be tested for MERS-CoV as advised in the current surveillance recommendations.
Specimens from patients’ lower respiratory tracts should be obtained for diagnosis where possible. Clinicians are reminded that MERS-CoV infection should be considered even with atypical signs and symptoms, such as diarrhoea, in patients who are immunocompromised.
Health care facilities are reminded of the importance of systematic implementation of infection prevention and control (IPC). Health care facilities that provide care for patients suspected or confirmed with MERS-CoV infection should take appropriate measures to decrease the risk of transmission of the virus to other patients, health care workers and visitors.
All Member States are reminded to promptly assess and notify WHO of any new case of infection with MERS-CoV, along with information about potential exposures that may have resulted in infection and a description of the clinical course. Investigation into the source of exposure should promptly be initiated to identify the mode of exposure, so that further transmission of the virus can be prevented.
WHO does not advise special screening at points of entry with regard to this event nor does it currently recommend the application of any travel or trade restrictions.
WHO has convened an Emergency Committee under the International Health Regulations (IHR) to advise the Director-General on the status of the current situation. The Emergency Committee, which comprises international experts from all WHO Regions, unanimously advised that, with the information now available, and using a risk-assessment approach, the conditions for a Public Health Emergency of International Concern (PHEIC) have not at present been met.
