Bird Flu Spreads to Most Bali Districts; Lombok on Edge Over Poultry Deaths

Denpasar/Mataram. Health authorities in Bali said on Friday that the bird flu virus that killed two children there last week had spread to eight of nine districts on the island, while residents in neighboring Lombok are reporting similar chicken deaths.

Putu Sumantra, head of the Bali Livestock Agency, said that only Gianyar district had not reported cases of poultry dying from the disease.

“This is quite surprising, especially given how quickly the disease is spreading,” he said.

He added that the pattern of the spread of the disease was similar, although smaller in scale, to 2007, when a bird flu pandemic swept the island.

“Back then, around 4,000 birds died, but this time the figure is much smaller,” Sumantra said. “Nevertheless, the deaths of the two children is a very shocking thing to come to terms with.”

Almost 2,800 birds are confirmed to have contracted avian influenza. More than 1,300 died from the disease and a further 1,200 were culled.

“Most of the cases occurred in poultry raised on small farms,” Sumantra said. “Those on larger commercial farms tend to be vaccinated on a regular basis so aren’t as vulnerable.”

Ketut Kariyasa Adnyana, a member of the provincial legislature, expressed concern that the spread of the disease would hurt the island’s all-important tourism industry.

He also said it came at a particularly critical time, with US President Barack Obama among the clutch of world leaders scheduled to arrive in Bali for the East Asia Summit.

“The provincial and district authorities should take immediate steps to address this problem. We can’t afford to let it get out of hand,” he said.

On the neighboring tourist island of Lombok, sudden poultry deaths attributed to bird flu are also mounting.

Hundreds of chickens died on Friday at an Army dormitory in Mataram, the provincial capital, residents reported. A veterinary team from Bali has been to the site to take back tissue samples to test for avian influenza.

Patriot, a resident, said he and his neighbors had immediately buried the dead birds for fear that they had died from bird flu and that the virus could spread.
However, Retno Prayudo Yanti, head of the municipal animal hospital, said initial tests showed the chickens had probably died from Newcastle disease.

The virus that causes Newcastle disease in birds poses no serious health threat to humans, although exposure to infected poultry can lead to mild flu-like symptoms.

“But even though the initial results suggest Newcastle disease, we still need further tests before we can rule out bird flu,” Retno said.

The provincial health office also said it was not taking any chances and was urging all residents with flu-like symptoms to see a doctor immediately.

“There haven’t been any confirmed cases of bird flu among humans here recently, but this area is endemic for the disease so we have to be careful,” a spokesman for the office said.     http://www.thejakartaglobe.com/home/bird-flu-spreads-to-most-bali-districts-lombok-on-edge-over-poultry-deaths/471804

Abstract

Current egg-based influenza vaccine production technology can't promptly meet the global demand during an influenza pandemic as shown in the 2009 H1N1 pandemic. Moreover, its manufacturing capacity would be vulnerable during pandemics caused by highly pathogenic avian influenza viruses. Therefore, vaccine production using mammalian cell technology is becoming attractive. Current influenza H5N1 vaccine strain (NIBRG-14), a reassortant virus between A/Vietnam/1194/2004 (H5N1) virus and egg-adapted high-growth A/PR/8/1934 virus, could grow efficiently in eggs and MDCK cells but not Vero cells which is the most popular cell line for manufacturing human vaccines. After serial passages and plaque purifications of the NIBRG-14 vaccine virus in Vero cells, one high-growth virus strain (Vero-15) was generated and can grow over 108 TCID50/ml.

In conclusion, one high-growth H5N1 vaccine virus was generated in Vero cells, which can be used to manufacture influenza H5N1 vaccines and prepare reassortant vaccine viruses for other influenza A subtypes.

JVI: H5N1-SeroDetect EIA and Rapid Test: A Novel Differential Diagnostic Assay for Serodiagnosis of H5N1 Infections and Surveillance

arrow
ABSTRACT

Continuing evolution of highly pathogenic (HP) H5N1 influenza viruses in wild birds with transmissions to domestic poultry and humans poses a pandemic threat. There is an urgent need for a simple and rapid serological diagnostic assay which can differentiate between antibodies to seasonal and H5N1 strains that could provide surveillance tools not dependent on virus isolation and nucleic acid technologies. Here we describe the establishment of H5N1 SeroDetect ELISA and rapid test assays based on three peptides in HA2 (488-516), PB1-F2 (2-75), and M2e (2-24) that are highly conserved within H5N1 strains. These peptides were identified by antibody repertoire analyses of H5N1 survivors in Vietnam using whole genome phage display libraries (GFPDL). To date, both platforms demonstrated high levels of sensitivity and specificity in detecting H5N1 infections (clade 1 and clade 2.3.4) in Vietnamese patients as early as 7 days and up to several years post infection. H5N1 uninfected individuals in Vietnam and the US, including subjects vaccinated with seasonal influenza vaccines or confirmed seasonal infections, did not react in the H5N1-SeroDetect assays. Moreover, sera from individuals vaccinated with H5N1 subunit vaccine with moderate anti-H5N1 neutralizing antibody titers did not react positive in the H5N1-SeroDetect ELISA or rapid test assays.

The simple H5N1-SeroDetect ELISA and rapid tests could provide an important tool for large scale surveillance for potential exposure to HP H5N1 strains in both humans and birds.

Mataram, 14/10 (AFP) - Medical experts and paramedics at the General Hospital of West Nusa Tenggara Province, increasing the observation of patients who show symptoms of the virus of bird flu or Avian Influenza. "Observations of bird flu patient's symptoms improved to anticipate the spread of bird flu from the island of Bali," said Director of the Provincial General Hospital (Dr) West Nusa Tenggara (NTB) dr H Then Mawardi Hamri, who was accompanied by Head of Service, dr H Hamzi Then, in Mataram, Friday.
He said, every patient who entered the department of NTB will be watched carefully especially those showing symptoms of bird flu, such as high fever, respiratory and abdominal complaints.
Patients with symptoms will be immediately given the antiviral drugs H5N1 or type of drug that can inhibit the replication of neuramidase (neuramidase inhibitors), such as Oseltamivir (tamiflu) and zanamivir. However, antiviral drugs that have side effects and should be given within a certain time so that the necessary opinion doctor.
"In addition to the hospital, antiviral drugs is also provided at various health centers to be easily accessible to patients with symptoms of bird flu. Stock is still a lot and when we asked again out in the center," he said.
Mawardi said, serious observation of the patient's symptoms of bird flu was carried out at the entrance to patient care as in intalasi Emergency Department (ER), and other treatment rooms, but especially new patients because the patients had been diagnosed a long course.
Especially for referral of patients who suffered from bird flu, can be directly taken to the installation space on the second floor of the department of NTB, without having to pass through the ER room.
"We are ready to serve patients of bird flu, as the only hospital in the province who berkewenangan handle bird flu patient referrals," he said.
According to Mawardi, since 2009 the department of NTB have a special team handling bird flu patients consisted of specialist physicians, such as pulmonary specialists, ENT, children, medicine (internist) and radiology.
Special team handling bird flu can not be dissolved because until now Indonesia still categorized countries 'suspect' bird flu. In fact, more and more areas have been identified bird flu as Bali, a neighboring province.
RSUP NTB has also supported a number of special equipment handling bird flu patients, but has not been used because not a single bird flu positive patients, except for some who are "suspect" or unexpected.
Isolation rooms are also available, but has been used for other patients who need care in a special room.
"So, we are ready to serve patients of bird flu. From the aspect of human resources, equipment and isolation rooms are available," says Mawardi.
Mawardi admit, a serious observation of patients with symptoms of bird flu was considered important because Bali has been infected, and there are indications the spread of bird flu on the island of Lombok, which is known from the results of laboratory analysis of blood sempel hundreds of chickens that died suddenly at the Praya and Pringgarata, District Central Lombok, since last week.

We reported an article earlier here, which spoke of a denial of bird flu patients at their hospital. From the article:
Director of the Provincial General Hospital (Dr) NTB, dr. H. Mawardi Hamri, denied, the issue of bird flu patients are being treated there. Since the year 2009, there were no patients were affected by the H5N1 virus in NTB. Formerly there were patients who had isolated in special rooms, for allegedly affected suspect bird flu. But after the blood test, he was declared negative, so it sent back.

"If there are patients in suspected bird flu virus, then the Dr will immediately respond and provide services in accordance with Standard Operational Procedure (SOP) of bird flu. NTB RSUP ready to serve whenever the bird flu patients, "said dr. H. Mawardi Hamri, accompanied by Head of Medical Services and Support, dr. H. L. Hamzi F, told reporters in his office on Friday (14/10).

Naturally, the patients will test negative, because as part of the Standard Office Procedure, they will put them on Tamiflu as soon as they spot the very first symptom of bird flu, H5N1.

Indonesia: Bangli Hospital CEO: "We still do observation for 7 days to 32 Health Workers"

October 15, 2011
Bangli hospitals are not prepared to do the bird flu positive patient care. For the legality of the various requirements and necessary care determined by ministerial letter. To Bali defined three special hospitals that treat patients with diseases of this type, Tabanan Hospital, Gianyar and Sanglah Hospital. Hospital CEO Bangli dr. I Wayan Sudiana, M. Kes., Friday (14/10) yesterday.

According to him, specialty hospitals treating victims of bird flu has available special isolation rooms and equipment. So even with the officers who are trained and prepared with virus protective equipment. ''This is not owned hospitals Bangli. If you find any indication or history of symptoms of this disease type it directly refers to the Sanglah Hospital,''he said.

As the incident that occurred recently. Self-protection equipment in hospitals Bangli just enough for two people. Logically, if the patient was referred to Sanglah Hospital, just enough for one nurse and one ambulance driver.

Hospital CEO Bangli dr.
I Wayan Sudiana: ''We still do observation for seven days to 32 health workers who could take care of our second bird flu victim. Thank God for our health workers are still alive. Because they do when treating not equipped with self-protection. Let alone in hospitals Bangli, positive lab tests to determine whether patients infected with RS Sanglah still do beberapakali testing with other institutions,'' he said

In the future it also expects the cooperation with the Department of Animal Husbandry and Fisheries Bangli. He expects, this service provides a list of everywhere in the region Bangli threatened and already infected with appropriate rapid test results. That way if there were patients from areas that have been defined as the zone of infection, treatment can be done to the fullest. Including health care workers protect themselves alacrity. ''Our officers than there has been no adequate facilities for the handling of bird flu. Officers are also not trained for it,'' he admitted.

Market Banyuasri team found five tails range chicken (chicken) died suddenly. Having tested by rapid test (rapid test) chickens that died suddenly was declared free of bird flu.

Head of Department of Agriculture and Livestock (Kadistanak) Buleleng Regency, Ir. Putu Mertha Jiwa, THAT was accompanied by Head of Animal Health (Keswan), Wayan Vice Squad, said the birds with a disinfectant spray is done to prevent avian flu viruses infect birds. Reasons targeting poultry traders and collectors, as in the traditional market is where the circulation of birds from various villages in Buleleng.

In fact, outside Buleleng suspected poultry are also many in the traditional markets in Buleleng. To that end, Distanak suggest traders and collectors do not bring in poultry from outside the region. Therefore, poultry imported from outside the area or from areas that have been infected with bird flu is very difficult diteksi. ''We continue to provide insight to merchants to get together to be alert because the virus is very difficult to detect. We also recommend traders to sell and wear a mask when cleaning the cage sanitation,'' he said.

On the other side, Mertha Jiwa said domestic poultry were found dead when the team doing the spraying declared bird flu negative. This is corroborated by the results of rapid tests conducted showed domestic poultry that died was not because of bird flu, but suspected because of the heat or trampled by the other chickens in the cage. ''We've done the test through saliva and feces of chickens that died, the result is negative and we conclude that chickens died because of other factors,''he explained. (KMB)
