Nationwide swine flu death jumps to 1,113

Genetic Analyses of Influenza in Wild Birds Can Improve Avian Flu Surveillance Programs

ScienceDaily (Jan. 12, 2010) — Genetic analyses of avian influenza in wild birds can help pinpoint likely carrier species and geographic hot spots where Eurasian viruses would be most likely to enter North America, according to new U.S. Geological Survey research.

Persistence of the highly pathogenic avian influenza H5N1 (HPAI H5N1) virus in Eurasia and Africa, and concerns that the virus might be transported among continents by migratory birds has resulted in global surveillance programs. In the United States, state and federal agencies tested more than 326,000 wild bird samples from across the country from 2005 to 2008.

The new work by USGS has nationwide importance because it offers a method for avian influenza surveillance programs to target their efforts for the right species and in the best locations.

In the study, USGS scientists conducted the first-ever survey of avian influenza gene variation in a single host species -- the northern pintail -- at each end of the bird's migratory flyway in North America: Alaska and California. These birds migrate between North America and Eurasia and in Japan and China have been known to occur in outbreak areas of HPAI H5N1.

The researchers discovered that some avian influenza viruses recovered from the North American pintails contain genes that are more closely related to influenza viruses in Eurasia, and that the greatest number of these genes occurred in pintail viruses from Alaska. In contrast, northern pintails sampled on their main wintering areas in California had few Eurasian virus genes.

The researchers speculate that Euasian flu genes become less prevalent as birds migrate southward in fall due to rapid mutation and reassortment, common to influenza viruses, and dilution by existing North American flu viruses. Reassortment, a shuffling process among viruses that infect the same host, occurs in all types of influenza A viruses, including H1N1 and H5N1.

"Our research demonstrates a genetically based technique for prioritizing wild bird species that are targeted for surveillance," said Dr. John Pearce, a USGS scientist and lead author of the study. "Refining the list of priority species for surveillance by this method can reduce time and effort involved in surveillance sampling and is needed not only for Alaska, but also for those species along the North

Atlantic coast of North America that may engage in transcontinental migrations, such as shorebirds and gulls," Pearce said.

With few exceptions, genetic evidence for transcontinental avian influenza virus exchange in North America has come from coastal regions closest to Europe or Asia -- Alaska and the North Atlantic.

These areas, said Pearce, probably represent the first or primary areas of contact for foreign viruses, yet only about a third of birds tested for HPAI H5N1 in the United States so far have been from these regions.

"Based on this new genetic evidence, one possible new strategy would be to target surveillance efforts on species in these coastal regions that are geographically closer to current sources of the highly pathogenic H5N1 virus," Pearce said. "If there is no evidence of transcontinental avian influenza virus gene exchange for a certain species or regional pathway, then those species and areas could be deemphasized in future surveillance programs."

The research was published in the November 2009 issue of Evolutionary Applications, and was authored by scientists from three USGS centers: the USGS Alaska Science Center, the USGS National Wildlife Health Center, and the USGS Western Ecological Research Center.

"We expect and indeed welcome criticism and a chance to discuss it," WHO spokeswoman Fadela Chaib told journalists.

"In the future we will also do an evaluation of our work with outside experts, in order to assess the work of WHO in the management of the pandemic influenza H1N1," she added.

"Evaluation is necessary and we are open to recommendations that can improve our work, when this is over WHO will undoubtedly work with outside experts."

The UN health agency was still working out the details of such a process.

Council of Europe parliamentarian Wolfgang Wodarg last week said he and several colleagues had called for a commission of inquiry into a "false pandemic" and the way it was handled at national and European levels, claiming pressure from pharmaceutical firms.

Some European countries have been trying to sell off stocks of unused vaccine in recent weeks, after many had geared up to deliver two doses instead of what eventually turned out to be the required single dose.

Chaib denied that the WHO was in conflict with its member states.

WHO flu spokeswoman Nyka Alexander said it could take "several seasons" to determine that a pandemic was over.

"We can't say a pandemic is over until we have seen that it hasn't come back," she explained.

FDA advises on manufacturing MNPs during a pandemic

Continuing production of medically necessary products (MNPs) in the event of a pandemic will be made easier by developing a contingency plan, according to the FDA which has issued guidance on the subject.

The high profile of H1N1, and earlier threats from H5N1 and SARS (severe acute respiratory syndrome), have raised awareness of the damage widespread employee illness could cause to pharma companies’ manufacturing operations.

To mitigate against this threat the US Food and Drug Administration (FDA) recommends manufacturers of MNPs formulate a plan. MNPs are defined as serious disease treatments which cannot be substituted with an alternative product.

In its draft guidance the FDA states that the plan should be developed and tested before a crisis occurs. Failure to do so could lead to companies developing plans during a pandemic, when there may be insufficient time and management resources.

Furthermore, the agency recommends that if manufacturers can foresee a period of high absenteeism they should begin preparations. These steps include increasing the inventory of MNPs and raw materials, training personnel for different roles and conducting maintenance.

The guidance lists periodic maintenance as one of the activities that could be ceased in the event of high absenteeism, along with some stability testing, environmental monitoring of facilities and examinations of data.

Making these cutbacks is intended to focus resources on essential manufacturing steps. If these are insufficient the FDA suggests companies could reduce second-person verification activities, delay deviation investigations of minor events and cease in-process adequacy of mix tests.

Implementing these measures, and others listed in the guidance, could result in the manufacturing process failing to meet all statutory and regulatory requirements.

However, the Center for Drug Evaluation and Research (CDER) “is prepared to exercise enforcement discretion” in the event of high absenteeism, as long “as the product remains safe and effective”.

To ensure this goal is achieved the FDA recommends that manufacturers carefully monitor indictors of product quality to note unfavourable trends. Furthermore, manufacturers should keep samples for testing at a later date if it cannot be performed at the time.

A leading health expert said the swine flu scare was a "false pandemic" led by drug companies that stood to make billions from vaccines, The Sun reported Monday.

Wodarg said, "It's just a normal kind of flu. It does not cause a tenth of deaths caused by the classic seasonal flu.

"The great campaign of panic we have seen provided a golden opportunity for representatives from labs who knew they would hit the jackpot in the case of a pandemic being declared.

"We want to clarify everything that brought about this massive operation of disinformation. We want to know who made decisions, on the basis of what evidence, and precisely how the influence of the pharmaceutical industry came to bear on the decision-making."

He added: "A group of people in the WHO is associated very closely with the pharmaceutical industry."

The WHO recently reaffirmed its stance that the pandemic is not over. However, the number of swine flu deaths is dramatically lower than expected.

In an interview with France's L'Humanite Sunday, Wodarg also raised concerns about swine flu vaccines.

"The vaccines were developed too quickly. Some ingredients were insufficiently tested," he said.

"But there is worse to come. The vaccine developed by Novartis was produced in a bioreactor from cancerous cells, a technique that had never been used until now.

"This was not necessary. It has also led to a considerable mismanagement of public money.

"The time has come at last for us to make demands on governments. The purpose of the inquiry is to prevent more false alarms of this type in the future."

"We must make sure people can rely on the analysis and the expertise of national and international public institutions. The latter are now discredited, because millions of people have been vaccinated with products with inherent possible health risks."

Sanofi’s Vaccine for Mosquito Virus Protects Adults, Study Says

Jan. 11 (Bloomberg) -- Sanofi-Aventis SA’s experimental vaccine against dengue protected healthy volunteers against all four strains of the virus in a study, bringing the drugmaker closer to providing the first vaccine against a disease that threatens 40 percent of the world’s population.

The vaccine protected all 66 adults a U.S. study in which they were given three shots over 15 months, according to the results published Jan. 9 on the Journal of Infectious Diseases Web site. Most people were protected after two doses, including those who received a placebo for the first injection and the vaccine for the second and third shots, the study showed.

Sanofi is conducting a larger trial that includes children, who are more likely to become sick or die from dengue. Paris- based Sanofi said in May it will invest 350 million euros ($508 million) to build a plant to make dengue vaccine in France, preparing for demand that analysts have said could generate annual market revenue of about $1 billion.

The study “is an important step,” the researchers wrote in the trial funded by Sanofi. “These results paved the way for ongoing and future large-scale efficacy trials of our candidate.”

There is no specific treatment for dengue. The most common side effects in the study were headache, malaise and muscle pain, the researchers said.

About 2.5 billion people worldwide live in areas prone to the mosquito-borne viruses, which the World Health Organization says causes about 50 million infections each year. Mosquitoes capable of transmitting the virus are now found in 28 U.S. states, according to a 2009 study.

The U.S. government, which increased its vaccine order by 29 percent in September, is running advertisements in national newspapers featuring an open letter urging Americans to get the vaccine.

“The willingness to be vaccinated against swine flu is much lower than what governments projected,” said Andrew Goodsall, a health-care analyst with UBS AG in Sydney. Goodsall estimates the smaller order will trim as much as A$30 million ($28 million) from CSL’s full-year net income.

The U.S. is allowed to change the quantity of its order under its contract with CSL, David said. The company’s contracts with other governments, including Australia and Singapore, don’t contain so-called stop work orders, she said. Excess vaccine material will be used in the production of seasonal flu shots, David said.

Financial Impact

The “financial impact cannot be known until the pandemic and seasonal programs have run their course, but is not expected to be major,” she said.

CSL fell 18 cents, or 0.6 percent, to A$31.93 on the Australian stock exchange today, compared with a 0.8 percent increase in the benchmark S&P/ASX 200 Index.

What animals can help spread flu viruses? Two wildlife disease experts examined which species might act as reservoirs for influenza viruses and be "mixing vessels" in which new strains are generated. They analyzed complex carbohydrates found on the surface membranes of cells, which can act as "receptors" for influenza viruses.

Mark Schrenzel and Bruce Rideout already knew H5N1, the avian strain of influenza, makes use of receptors containing an alpha 2,3 sialic-acid linkage, whereas H1N1 strains employ the alpha 2,6 sialic-acid bond. They also knew any species (domesticated swine being one) that carries receptors of both types could act as a mixing vessel. They set out to find which animals have what, focusing on species likely to meet waterfowl like geese and ducks, known to be vulnerable to H5N1. The researchers analyzed the receptors of 60 species.

Pekanbaru Teen in Critical Condition, Coma After Contracting Bird Flu

A17-year-old boy from Pekanbaru, Riau, is now in a critical condition after being infected with the deadly bird flu virus.

“The patient is in a critical condition and in a coma,” said Azisman Saad, head of the bird flu team at Arifin Achmad General Hospital in Pekanbaru.

Azisman said the boy, identified only as JF, was isolated in intensive care after suffering high fever and trouble breathing.

“He needs a breathing machine to survive,” Azisman said, adding that the provincial health agency had sent the boy’s blood sample to the Ministry of Health and the test result came back positive for H5N1 avian influenza.

Evy, one of the boy’s relatives, said the family did not know where JF could have contracted the virus because he had no history of direct contact with sick poultry.

Evy said JF fell into coma on Friday after experiencing some bird flu symptoms, including trouble breathing, fever and diarrhea.

Azisman said the hospital had handled two other positive cases of bird flu since December, both occurring in children below the age of 5. One of the patients died on Dec. 31, while the other was recovering and had been released from isolation.

Azisman said a 5-year-old girl from Indragiri Hulu died because she received treatment too late and was brought to the hospital in a very bad condition.

Tjandra Yoga Aditama, director general of communicable diseases at the Health Ministry, said 20 people had been confirmed as having contracted bird flu in 2009, of which 19 had died.

The findings meant Indonesia remained the country with the highest number of bird flu cases and fatalities. Since the outbreak first emerged in 2003, 137 people have died in Indonesia, while the virus has killed less than 300 people worldwide.

Pekanbaru (ANTARA News) - A boy initials JF (17), who allegedly (suspect) infected with the virus that causes bird flu, H5N1, is now in critical condition at the Arifin Achmad Hospital in Pekanbaru, Riau.

"Patients in critical condition because until now still in a coma," said Chairman of Avian Influenza Control Arifin Achmad Hospital, Azisman Saad, told AFP in Pekanbaru, Sunday.

Pekanbaru residents were still in intensive care in a special isolation room bird flu patients. According to Azisman, patients experience a high fever with body temperature reached 37.5 degrees Celsius.

In addition, he said, patients also experience difficulty in breathing because breath reaches 60 per minute. Meanwhile, breathing human beings under normal conditions range from 16-20 per minute.
